must have actual state license board certification in hand, and
active DEA for the state of application and CSR if needed per state
in hand. Physician Only coverage needed Coverage model: Physician:
0600 to 1800 NPPA: 1200 to 2300 Physician: 1800 to 0600 Need:
Looking for end of May start, both days and nights Physician
Requirements Summary AR MD License, AR DEA License, Medical Degree,
Residency Certificate, Board Certification not required but if not
Board Certified must show 2500 HR's in an ED over past 5 years,
ACLS, PALS and ATLS, 2 Trauma CME's every three years, 2 years case
logs, 2 years CME's, Immunization Record W. Hep B., Previous TB
Test, Current Flu Shot, Current Malpractice, Past 2 years of CME's,
Past 5 years of malpractice, history of all affiliations, Color
Photo, SS Card. * ATLS, ACLS, BLS * ER experience preferred * Not
required to have ABEM. * 125 Hospital Beds; 13 ED * 24,500 Annual
ED Volume * Level III Trauma Center * T-System EMR Schedule Days,
